Your annual visit matters!

Through annual wellness exam and routine health screenings, your provider can determine your current health status and detect early warning signs of more serious problems. Your annual/preventive care services may include immunizations, physical exams, lab work and x-rays.

The Annual Wellness Visit (AWV) is a yearly appointment with your primary care provider (PCP) to create or update a personalized prevention plan. This plan may help prevent illness based on your current health and risk factors.

Annual Wellness Visit (AWV) is covered under basic insurance plans including Medicare and Medicaid.

During your wellness exam with your PCP, you may receive screening for cholesterol, blood pressure, diabetes, mammogram, pap smear, osteoporosis, or STDs. Your PCP may ask you about current stress level, physical activities, diet, or drug use such as tobacco and alcohol.

Our providers value the importance of routine physicals for patients. During annual exams, patients meet with a board-certified provider who will provide a thorough examination and review of the patient's medical history. Coupled with the appropriate diagnostics and lab work, provider will evaluate your health. The provider will discuss the test results so you can ask questions and bring up any concerns you may have. Annual wellness Visits can be scheduled from patient portal. You can request an appointment for annual exams, sports physicals, back-to-school physicals, post-operative examinations and job-related physicals.
