Website Privacy Policy

Scope

This policy covers the public website of Primary Care Services, S.C. (Primary Care Services). It does not replace our Notice of Privacy Practices, which explains how we use and protect your health information as a patient.

Please do not send urgent or sensitive health information

The website contact form is for non-urgent questions only. Do not include medical details, test results or other sensitive information in it. For urgent medical concerns call (630) 429-9000. For emergencies call 911.

Contact form

If you use the contact form, we receive the name, email address, phone number, subject and message you type. We use them only to reply to you. The message is delivered to our staff by email through an email delivery service we use for this purpose.

The form is protected by Cloudflare Turnstile, a security check that tells people from automated programs. To do this, Cloudflare processes technical signals from your browser, such as your IP address. This is described in Cloudflare's own privacy policy.

Website analytics

We use Cloudflare Web Analytics to understand how many people visit which pages. It is designed to work without cookies and without tracking you across other websites, and we do not use it to identify individuals. We do not send it anything you type into a form, and we do not use advertising or marketing trackers. If your browser sends a Do Not Track or Global Privacy Control signal, we do not load analytics at all.

Other services this site uses

• Fonts are loaded from Google Fonts, so your browser contacts Google to download them.
• Embedded maps on our location pages are provided by Google Maps and are subject to Google's terms.
• Your language choice (English or Spanish) is saved in your own browser so the site remembers it.
• The patient portal and online scheduling are separate services (Epic MyChart and our scheduling page). When you leave this website to use them, their own privacy terms apply.
