Primary Care Services is a patient-centered medical home for adults. Your primary care provider is your first call for new symptoms, ongoing conditions, preventive care and coordination with specialists, so your care stays connected over time.

How primary care works here

We are a Patient-Centered Medical Home (PCMH), which means care is organized around a long-term relationship with your clinician and a coordinated care team. Between visits you can message the care team, request refills and review results through the patient portal.

Getting started

New patients can book online or call (630) 429-9000. Please bring your insurance card, photo ID and a list of current medications. Forms can be completed ahead of your visit from our Forms page.
